The form should contain the following fields:

1. First Name (input box)
2. Last Name (input box)
3. Email (input box)
4. Topic (select box)
5. Category (select box)
6. Phone No. (input box)
7. City (select box)
8. Country (select box)
9. Website (optional) (text box)
10. Experience (input/ text box)
11. About Yourself (text box)

It should have enough space between the top and bottom such that any error can be shown.
